Chloride Shift

The chloride shift refers to the movement of chloride ions (Cl-) into and out of red blood cells (RBCs) and plasma. This phenomenon plays a crucial role in maintaining the acid-base balance in the body.

Process

- When carbon dioxide (CO2) is produced in tissues, it diffuses into RBCs and reacts with water to form carbonic acid (H2CO3).
- Carbonic anhydrase enzyme in RBCs catalyzes the conversion of carbonic acid into bicarbonate ions (HCO3-) and hydrogen ions (H+).
- The bicarbonate ions move out of RBCs into plasma in exchange for chloride ions (Cl-), which move into RBCs to maintain electroneutrality. This is known as the chloride shift.
- In the lungs, where CO2 is expelled, the process is reversed. Bicarbonate ions move back into RBCs in exchange for chloride ions, which move back into the plasma.
