A can't be correct as we all know that matter changes from 1 state to other in form of energy. b and c are wrong as negative charge change into negative and negative to positive. But the point is tha amount of negative or positive charges created are equal to the amount of negative or positive charge decrease. For eg if there is increase of +1 proton there will be decrease of - 1 electron simultaneously. Thus net charge will be constant. Hence d is correct.
