The main reasons behind the Bhopal Gas tragedy are-->

4. unawareness among the workers and labours.

It was caused because water entered a tank containing Methyl isocyanate. And because of it thousands of people died because of leakage of gas and now also in that many people have breathing problem or they are handicapped and I think that the main reason behind this was that the workers were not aware of what reactions can happen from water and gases.
