Urea is a waste product that is formed in the liver during protein metabolism. It is filtered by the glomerulus and then reabsorbed in small amounts in the proximal tubule. Some urea also passes from the collecting duct into the medullary interstitium and enters the ascending limb of the loop of Henle. This recycling of urea helps in the maintenance of the concentration gradient in the medullary interstitium, which is important for the reabsorption of water in the collecting duct.

Water reabsorption in PCT:
The majority of water reabsorption occurs in the proximal convoluted tubule (PCT). Around 65% of filtered water is reabsorbed in the PCT through a process called passive osmosis. This occurs due to the presence of water channels called aquaporins in the apical and basolateral membranes of the PCT cells. The reabsorption of water in the PCT is driven by the osmotic gradient created by the reabsorption of solutes, such as sodium, glucose, amino acids, and bicarbonate.

Vasopressin and water reabsorption:
Vasopressin, also known as antidiuretic hormone (ADH), plays a crucial role in regulating water reabsorption in the kidneys. It acts on the collecting ducts and the distal convoluted tubules to increase the permeability of their walls to water. This allows more water to be reabsorbed into the bloodstream, resulting in concentrated urine. However, vasopressin does not directly affect water reabsorption in the PCT.

ADH or antidiuretic hormones are secreted from the neurohypophysis. This hormone makes the cells in the wall of collecting ducts permeable to water. Due to this, water moves from tubular fluid into tissue fluid, making the urine concentrated.
When there is any change in the osmolarity of our blood, ADH stimulates the reabsorption of water from the last part of DCT and the entire collecting duct by increasing the permeability of cells. Eventually, there is a decrease in the osmolarity of blood.
